8 Year Results After Ridge Augmentation With Customized Allograft Bone Blocks

Patients with severe ridge atrophy who want to have an implant-supported restoration may benefit from individualized treatment protocols. Allogenic bone blocks as well as autogenous grafts are the materials of choice in many of these cases. The success or failure of the grafting procedure is determined by the close contact of the graft to the recipient bone surface.

Customized Allograft Blocks are a crucial developmental step in block grafting. A custom block is produced using CAD/CAM technology based on a CBCT/CT scan of the defect area. The preoperative design and fabrication of the individual bone graft reduces the necessity to shape the graft during surgery. This optimization of the procedure reduces the surgical time significantly, leading to less complications.

This lecture focuses on the history and experiences of allograft block grafting in dental surgery. Based on photo and video documentations, histological results and clinical evidence it will be shown why customized allograft bone blocks are a valuable alternative to autogenous bone grafts.

The emergence profile is a crucial factor to facilitate favorable esthetic outcomes and maintain peri-implant health and stability. It should be evaluated not simply in terms of morphology but as a clinical variable closely related to materials properties, surface chemistry and clinical approaches; in this way, it is possible to achieve a comprehensive planning of this critical factor that can influence significantly the integration, stability and preservation of bone and soft tissues. Soft tissues integration and adherence onto prosthetic components represent paramount elements to preserve and protect bone from contamination and infection. Zygomatic Implant Redesigned: Rationale and Biomechanical Principles

The evaluation of patients for immediate loading who have with terminal dentition, especially in the maxilla, may at times be challenging. The presence of pneumatized sinuses limits the surgical options for placement of implants for posterior support. As always, treatment planning begins with the “end in mind.” The Determination of the type of final prosthesis, whether a tooth only or hybrid bridge is essential to guide the surgical team in planning the number as well as the distribution of the implants needed along the arch length. The Adoption of the “Zones of the Maxilla”, allows the surgical team to determine whether to use axil, tilted or the zygoma implant. In cases where the Zygoma or the Quad Zygoma is planned, the adoption of the “ZAGA Classification” allows the surgeon to evaluate the contour of the lateral wall of the maxillary sinus as well as the extent of palatal resorption of the maxillary alveolar ridge.

In this presentation, an algorithm to ensure the ability to obtain initial stability of implants in the posterior maxilla will be discussed. The rationale for the modification of the Zygoma implant and its application will also be reviewed. The New Straumann Round and Flat Zygomatic implants address the potential limitations that have been observed by experienced surgeons over the last two decades using zygoma implants. Our protocol for the digital immediate loading with terminal dentition cases includes the extraction of all non-salvageable teeth, implant placements, abutment placements and the intraoral scan of the freshly placed abutments, computer aided designs and milling.

Zirconia in Implant Dentistry – An Update

Zirconium dioxide (zirconia) has become a highly popular material for tooth- and implant-supported restorations. Among the many advantages are the optical and physical properties and arguably even more so, the biological integration, which is especially important for dental implant restorations. In addition, CAD/CAM technologies provide excellent accuracy and precision of fit. However, there are still many questions related to the advantages but also limitations of zirconia, such as: can zirconia be used like a metal, is all zirconia the same, what are the best protocols for zirconia veneering, what are the different types of zirconia and how to select them, how about precision of fit, and what is the soft-tissue response? These questions become even more important for long-span zirconia fixed dental prostheses. Among the most recent developments in this area is the new Nobel Procera Implant bridge, which will be introduced.

This presentation will provide an update zirconia as it is applied in implant dentistry on single-unit restorations up to full-mouth reconstructions and introduce new material options.

A New Solution for Customized Bone Regeneration for Complex Alveolar Ridge Defects

One of the greatest challenges facing clinicians is to place implants in an ideal position in sites compromised by trauma, periodontal disease and/or severe infection, where significant bone augmentation is necessary. Recent studies have confirmed that implants, placed after vertical augmentation by means of Titanium Meshes and followed by an adequate supportive care program, offer predictable long-term results. This program will show that careful selection of a proper bone-grafting technique and soft tissue augmentation can produce good functional and esthetic results.
